Insurance

Sarah is a preferred provider for Aetna, Blue Cross Blue Shield, and Cigna. Please call your insurance company before coming to your first appointment to find out what your co-pay and deductible are. Please bring your insurance card to your first appointment. If you are covered by another insurance company, we can provide you the necessary forms for you to file for reimbursement as an out-of-network provider. Contact your insurance company to find out what your plan covers for an out-of-network provider and what your deductible is for an out-of-network provider.

Private Rates

If you have no health insurance coverage, please talk to us about sliding scale options. If you cannot make an appointment please provide at least 24 hours advanced notice, otherwise you will be expected to pay for that session.

Payment

We accept cash, credit, debit, health savings account cards, personal check or money order. Payment is due at beginning of service.
